All Resumes must submitted by December 1, 2011 _______________________________________________________________________________________________________________________________________________________________________ ******Attention BLS First Response Agencies****** In an attempt to update records, the New York State Department of Health Bureau of Emergency Medical Services (BEMS) is requiring Basic Life Support First Response (BLSFR) agencies to complete the following packet: Until this packet is completed your agency will be ineligible for training reimbursement. If the completed and signed packet is not received by October 13, 2011, then BEMS will assume that your agency is not participating in 911 EMS response in your community and your four digit Department issued agency code will be deactivated. Below are some templates for the EMS Participation Agreement and collaborative agreements for PAD & Epinephrine auto injectors in Word format so you can edit as needed. Collaborative Agreement EPI PEN ___________________________________________________________________________________________________________________________________ Please be advised that the 2011 ALS Skills and CME Verification Form has been sent to all ALS agencies and providers via USPS. A few key notes about this year's form: · We have eliminated the “supplemental form” that required the collection of multiple signatures for those that work or volunteer under multiple Medical Directors. We now require simple documentation of agency affiliations on page 1 of the form. · Skill requirements have changed. The REMAC feels that the changes reflect where we are at with ACLS guidelines and the upcoming protocols. Please pay close attention to what is now required and read carefully the requirements for those that obtained intern or on-line status after July 1st. · As usual, copies of provider certifications must be submitted with the form. · Please note that ALL ALS providers must submit the form and supporting documents. This includes INTERNS. · CIC’s, CLI’s, Medical Directors and Medical Director Designees are allowed to evaluate and sign off on skills not performed in the field. The evaluator must have the same level or a higher level of certification. Skill evaluations MUST be documented and filed appropriately. NYS DOH EMS Practical Skills sheets are acceptable. Please contact your medical director for more information on their designees. · The provider and their primary agency are both responsible for maintaining an up to date and accurate file that includes but is not limited to, all supporting documents such as skill sheets, and CME certificates. Although PCR’s are stored at the agency where the call took place, we reserve the right to request copies as necessary for audit purposes. Random audits and checks will be performed to verify skills and CME’s. · The form and necessary supporting documentation must be submitted to the Mountain Lakes office by January 31, 2012. Late and or incomplete submissions will result in a suspension of your credentials and a $25 processing fee will be assessed. If suspended, you will have until February 28, 2012 to submit or fix your paperwork and pay the processing fee. If you fail to do so, your credentials will then be revoked.
